| Health care is undergoing
dramatic changes that create significant challenges for all.
The challenges will become even greater as the number of people
with chronic illnesses – conditions that require ongoing
attention and care – continues to rise. And increasing
economic pressures will further limit the ability of hospitals
and medical professionals to meet many of the needs of individuals
and families affected by chronic diseases.
How can we help these individuals and families
as more and more responsibility for managing their health
and health care falls on their shoulders? How can we give
them the information, resources, and support they need to
maintain their health and independence? How can we bridge
the gaps in our health system and create a continuum of care
that reaches from health education programs to assistance
with disease management to sensitive and humane end-of-life
care? One solution is to look to our faith communities. Churches
and other religious institutions, by working closely with
hospitals, medical professionals, and other organizations,
can play a vital role in creating a true continuum of care.

programs offered by the O’Neill Foundation for Community
Health and our Partners can help congregations and other caring
communities provide their members with the information and
support they need to maintain their health, dignity, and independence.
